Appointment Hours
11:00 AM to 6:00 PM –  Monday, Tuesday, Wednesday, and Thursday.

Hours are subject to change.

Please note that if you are late to your appointment, the session will still end at the scheduled time. If Ellen begins your appointment late or early, the session will run the full amount of time and end accordingly.

Payment
Cash, checks, most major credit cards, and Zelle are accepted. Payment is due at the time of appointment.  If you have forgotten to bring payment, we will gladly reschedule your appointment. Questions about fees and payment should be discussed prior to your appointment.

Insurance for Medical Nutrition Therapy (only)

IMPORTANT: E.L.M. Wellness is only in Network with Blue Cross Blue Shield. Please contact them to verify your Out-Patient Medical Nutrition Therapy benefits. Ask if codes 97802 and 97803 are covered and how many visits per year.  Also ask if you have a copay or need to meet a deductible before nutrition visits are covered. You may also want to inquire about coverage for virtual sessions. Not all insurance companies cover Telehealth for out-patient Medical Nutrition Therapy.

Ellen is a provider for the following BlueCross BlueShield of North Carolina plans:

  • Blue Options
  • Blue Advantage
  • Blue Select
  • Blue Options State Plan
  • PPO Blue Card Blue Care

Ellen is NOT a provider for the Blue Value Plan, Blue Local POS Plan, Blue Home Plan, Blue Care (HMO) Plan, or any other Government assisted plan.  

Ellen is NOT a provider for Medicare or Medicaid.

Cancellation Policy
24-hour cancellation notice by phone (not email or text) is respectfully required.  You will be charged for the missed appointment unless notice of cancellation is received 24 hours in advance of the scheduled time.  This fee is waived in the case of emergency hospitalization or natural disasters. The fee for missed appointments will be charged to your account.

If you repeatedly do not show for appointments or have repeated cancellations, this may result in termination of E.L.M. Wellness’s services.
