Appointment Hours
9:00 AM to 6:00 PM –  Monday through Friday.

Hours are subject to change.

Please note that if you are late to your appointment, the session will still end at the scheduled time. If your dietitian begins your appointment late or early, the session will run the full amount of time and end accordingly.

Schedule of Fees

Initial assessments are 80-minutes at a rate of $205.00.
Follow-up sessions are 50 minutes at a rate of $120.00.

Written reports requested by you or required by health professionals or insurance companies are $50.00.

Telephone consultations that are five minutes or less are complimentary, up to 30 minutes a rate of $60.00 will be applied, beyond 30 minutes the regular session rates will apply.

Fees are subject to change with a 30-day in office notice.

Cash, checks, most major credit cards are accepted.  We are a provider for the following BlueCross BlueShield of North Carolina plans:

  • Blue Options
  • Blue Advantage
  • Blue Select
  • Blue Options State Plan
  • PPO Blue Card Blue Care

We are NOT a provider for the Blue Value Plan.

Verify your health insurance coverage prior to arranging your first visit.  Specifically ask if procedure codes 97802, 97803, and 97804 are covered. Payment is due at time of appointment.  If you have forgotten to bring payment, Ellen will gladly reschedule your appointment. Questions about fees and payment should be discussed prior to your appointment.

Cancellation Policy
24-hour cancellation notice by phone (not email or text) is respectfully required.  You will be charged for the missed appointment unless notice of cancellation is received 24 hours in advance of the scheduled time.  This fee is waived in the case of emergency hospitalization or natural disasters. The fee for missed appointments is due within 7 days or at your next appointment, which ever comes first.

If you repeatedly do not show for appointments or have repeated cancellations, this may result in termination of E.L.M. Wellness’s services.
